Penile Fillers: What to Expect Before, During, and After
Non-surgical penile enhancement with hyaluronic acid (HA) filler has become one of the most sought-after procedures in men’s aesthetic medicine. For men considering the procedure, understanding what to expect at each stage — from the initial consultation through recovery — can help set realistic expectations and reduce anxiety. 1. The Consultation
Every reputable penile enhancement procedure starts with a thorough, one-on-one consultation. This is not a sales pitch — it is a medical evaluation. Your surgeon will assess your anatomy, discuss your goals, and determine whether you are a suitable candidate.
During the consultation, your physician will review your medical history, including any medications (particularly blood thinners), allergies, and prior genital surgeries. They will perform a physical examination and take standardized pre-procedure photographs. Importantly, the consultation is also an opportunity for the physician to screen for penile dysmorphic disorder — a condition in which a man with a normal-sized penis experiences significant distress about his size. Research has shown that the vast majority of men seeking penile enhancement have penises within the normal range, and setting realistic expectations is critical.1

2. Preparing for Your Procedure
Preparation for penile filler is straightforward. In the days leading up to your appointment, you should avoid blood-thinning medications and supplements (such as aspirin, ibuprofen, fish oil, and vitamin E) unless otherwise directed by your physician, as these can increase the risk of bruising. Alcohol should be avoided for at least 24 hours before the procedure.
No special fasting or sedation is required. The procedure is performed under local anaesthesia in an office setting, so you can eat normally and drive yourself to and from the appointment. You will be prescribed a short course of prophylactic antibiotics (typically Keflex 500mg, taken before and after the procedure) to minimise infection risk.
3. What Happens During the Procedure
The procedure itself takes approximately 30 minutes. Under the Urosculpt™ protocol, local anaesthesia is administered via a field block using 1% lidocaine (without epinephrine), which numbs the entire treatment area. Most patients report that the injection of the anaesthetic is the most uncomfortable part — once the area is numb, the filler placement is largely painless.
The filler is placed beneath the skin using a proprietary cannula-based technique rather than a sharp needle. The cannula approach significantly reduces the risk of vascular injury and bruising compared to needle-based methods. The physician works in a systematic pattern to ensure even, symmetrical distribution across the penile shaft.
A 2022 multi-centre randomised controlled trial found that hyaluronic acid filler produced a mean girth increase of approximately 22.7mm at 24 weeks post-injection, with high patient satisfaction.2
4. Immediate Results
One of the most appealing aspects of penile filler is that results are visible immediately. When you leave the clinic, you will see a noticeable increase in girth — both in the flaccid and erect states. There will be some initial swelling in the first 48 to 72 hours, which can make the result appear slightly more pronounced than the final outcome.
A large single-centre study of 324 patients reported a mean flaccid girth increase of 2.5cm (range 1.5–4cm), with 89% of patients reporting satisfaction with their results.3 It is important to understand that the filler adds girth (circumference), not length. Some patients do report a modest improvement in flaccid hang due to the added weight of the filler, but length enhancement is not the primary goal.
5. Recovery and Aftercare
Recovery is minimal. Most men return to desk work within one to two days. You should avoid sexual activity for seven days following the procedure to allow the filler to stabilise within the tissue. Strenuous exercise and heavy lifting should also be avoided during this period.
During the first three to five days, the filler remains somewhat mouldable. If you notice any asymmetry or unevenness during this window, gentle manual redistribution may be performed by your physician. After this period, the filler integrates with the surrounding tissue and is no longer manually adjustable.
Some mild bruising and swelling are normal and typically resolve within one to two weeks. Your physician will schedule a follow-up appointment to assess your results and address any concerns.
6. How Long Results Last
Hyaluronic acid is gradually metabolized by the body over time. The duration of results varies depending on the product used, the volume injected, and individual metabolic factors, but most patients can expect results to last approximately 18 to 24 months.
A 2011 study using Restylane Sub-Q for penile girth enhancement found that girth was maintained at 18 months post-injection, with minimal volume loss.4 More recent studies using newer-generation HA products have reported similar or improved longevity. Maintenance treatments can be performed to sustain results over time, and many patients find that subsequent treatments require less filler volume due to residual filler and collagen stimulation.
7. Safety and Reversibility
The safety profile of hyaluronic acid penile filler is well-documented. A comprehensive 2023 systematic review and meta-analysis found that HA produced greater girth enhancement and higher patient satisfaction than polylactic acid, with no serious adverse events reported.5
One of the key advantages of hyaluronic acid over other fillers (such as PMMA or silicone) is that it is fully reversible. If a patient is unhappy with the result or experiences a complication, the enzyme hyaluronidase can dissolve the filler rapidly. The most commonly reported complications are minor and manageable: temporary asymmetry, filler migration beneath the foreskin in uncircumcised men, and palpable nodules — all treatable with hyaluronidase. Infection rates are low, reported at approximately 1.5% in one large series, and typically managed with oral antibiotics.3
8. Who Is a Good Candidate
Good candidates for penile filler are men who are in good general health, have realistic expectations, and are seeking girth enhancement for aesthetic or confidence-related reasons. The procedure is appropriate for both circumcised and uncircumcised men.
Penile filler may not be appropriate for men with active genital infections, bleeding disorders, known hypersensitivity to hyaluronic acid, or significant psychiatric conditions such as body dysmorphic disorder. A thorough consultation with a qualified physician is essential.
